How to Read a Security Audit Report

You paid for a security audit. The report lands in your inbox — 40 pages of findings with severity ratings, technical descriptions, and proof-of-concept code. Now what?

Most teams skim the executive summary, create a few tickets, and move on. That's a waste of your investment. Here's how to actually read and use an audit report.

The Anatomy of an Audit Report

Standard Report Structure

Executive Summary (2-3 pages)

High-level overview for non-technical stakeholders. Overall risk rating, key statistics, and the most important takeaways. This is what your CEO/CTO should read.

Scope and Methodology (1-2 pages)

What was tested, what wasn't, how testing was performed, and any limitations encountered. Critical for understanding what the report does and doesn't cover.

Findings (bulk of report)

Individual vulnerabilities with severity, description, evidence, impact analysis, and remediation guidance. This is where the value lives.

Appendices

Detailed technical evidence, full request/response captures, tool outputs, and supporting data. Reference material for developers implementing fixes.

Understanding Severity Ratings

Severity ratings combine two factors:

Impact (What Can Happen)

  • Critical: Full data breach, RCE, complete account takeover, fund theft
  • High: Significant data access, privilege escalation, business logic bypass
  • Medium: Limited data exposure, non-critical function abuse, information disclosure
  • Low: Minor information leaks, best practice violations, theoretical risks

Exploitability (How Easy)

  • Easy: Exploitable by anyone with a browser — no authentication, no special tools
  • Moderate: Requires authentication or specific conditions, but straightforward with standard tools
  • Difficult: Requires chained exploits, race conditions, or specific environment factors
  • Theoretical: Possible in theory but requires unlikely conditions or significant effort

⚠️Don't Just Sort by Severity

A Medium-severity finding that's trivially exploitable may be more urgent than a High-severity finding that requires complex chaining. Prioritize by real-world exploitability, not just the severity label. As we discussed in why most security audits fail, failure to properly prioritize is one of the biggest reasons findings go unfixed.

Reading a Finding

Each finding should contain:

Title & Severity

What the vulnerability is and how serious it is

Description

Technical explanation of the vulnerability and the conditions required

Evidence / PoC

Proof that the vulnerability is real — screenshots, HTTP requests, exploit code

Remediation

Specific, actionable guidance on how to fix it

What to Look For

In the description:

  • Does it explain why this is a vulnerability in your specific context? (Not just a generic "XSS was found")
  • Does it describe the real-world impact? (What can an attacker actually do with this?)
  • Does it mention any prerequisites? (Authenticated user required? Specific browser?)

In the evidence:

  • Is there a working proof of concept? (If not, how was the finding verified?)
  • Can your developers reproduce it? (The PoC should be reproducible in your environment)
  • Does it show the full attack chain? (Some findings require multiple steps)

In the remediation:

  • Is the fix specific to your codebase and tech stack? (Not just "use parameterized queries")
  • Are there code examples? (The best reports include before/after code)
  • Are there any potential side effects of the fix? (Breaking changes, performance impact)

ℹ️Report Quality Is a Signal

If findings lack proof of concept, have generic descriptions, or offer vague remediation like "fix the vulnerability" — the audit quality may be poor. As we covered in how to choose a smart contract auditor, the report quality directly reflects the quality of the testing.

What to Do with the Report

Step 1: Debrief (Day 1)

Schedule a debrief call with the audit team and your engineering leads:

  • Walk through every critical and high finding
  • Ask questions about exploitability and real-world risk
  • Clarify remediation approaches for complex findings
  • Discuss findings the auditors considered but classified as acceptable risk

Step 2: Triage and Prioritize (Days 1-3)

Create tickets for every finding. Priority should factor in:

Prioritization Matrix

Fix immediately (P0)

Critical severity + easy to exploit. These are actively dangerous. Drop everything and fix them.

Fix this sprint (P1)

High severity or critical with complex exploitation. Important but can wait a few days for a proper fix.

Fix this cycle (P2)

Medium severity. Important for overall security posture but not immediately dangerous.

Track and plan (P3)

Low severity and informational. Best practices, hardening recommendations, and defense-in-depth improvements.

Step 3: Fix and Verify (Weeks 1-4)

  • Assign each finding to a developer with the right expertise
  • Write regression tests for each vulnerability before fixing it
  • Don't batch fixes — fix and verify each finding individually
  • Document the fix and the test in the ticket

Step 4: Retest (Week 4-5)

🛑Never Skip Retesting

A finding isn't "fixed" until the auditor confirms the fix works. Incomplete fixes are common — the obvious attack path is blocked but a slight variation still works. Always schedule retesting as part of the audit engagement.

Red Flags in Audit Reports

Signs the audit may not be thorough:

  • All findings are tool-generated — No manual testing, just scanner output
  • No proof of concept — Claims without evidence
  • Generic descriptions — Could apply to any application, not specifically yours
  • Missing finding classes — No business logic findings, no auth testing, no access control findings suggests limited manual testing
  • Very few findings — Every application has something. Zero findings means the audit was too shallow, not that the app is perfect.

For Web3: Additional Considerations

Smart contract audit reports differ from web app pentests:

  • Economic impact analysis — Findings should include potential financial loss (in dollar terms or percentage of TVL)
  • Proof-of-concept exploit code — Runnable Foundry/Hardhat test that demonstrates the exploit
  • Gas optimization notes — Lower severity but still valuable
  • Centralization risks — Admin key risks, upgrade authority, governance manipulation vectors
